全球專業實習聯盟平臺(以下簡稱本計畫)依據個人資料保護法(以下簡稱個資法)第八條第一項規定,向您告知下列事項,請詳閱:
一、本計畫因實習培訓與媒合等之目的而獲取您下列個人資料類別:姓名及連絡方式(包括但不限於電話號碼、傳真、E-MAIL或工作地址)等,或其他得以直接或間接識別您個人之資料。
二、蒐集之個人資料類別:識別類(C001、C003)
三、個人資料利用之期間、地區、對象及方式:
- 期間:本計畫因執行業務所必須之保存期間。 對象:與本活動業務相關之校內(外)單位、監督或主管機關。
- 地區:上述對象所在之地區。 方式:合於個人資料保護法及相關法令之利用方式。
四、依個人資料保護法第3條規定,可就您的個人資料向本計畫行使之下列權利:
- 查詢或請求閱覽。
- 請求製給複製本。
- 請求補充或更正。
- 請求停止蒐集、處理及利用。
- 請求刪除。
您因行使上述權利而導致對您的權益產生減損時,本計畫不負相關賠償責任。
五、您可自由選擇是否提供相關個人資料,惟您若拒絕或未依實或提供資料不完全,本計畫將無法為您提供特定目的之相關業務。
六、本計畫因業務需要而委託其他機關處理您的個人資料時,將善盡監督之責。
七、您瞭解此一同意書符合個人資料保護法及相關法規之要求,且同意本會留存此同意書,供日後 取出查驗。
八、個人資料之同意提供:
- 本人已充分知悉貴計畫上述告知事項。
- 本人同意貴計畫蒐集、處理、利用本人之個人資料,以及其他公務機關請求行政協助目的之提供。

In accordance with the Paragraph 1, Article 8 of the Personal Data Protection Act, the Global Internship
Facilitation of Taiwan platform (the “program”) must inform you of the following information:
1. For the purpose of internship training and placement, the program will collect the following
information from you: name, contact information (including, but not limited to, your phone number, fax
number, email address, and employment address), and other directly or indirectly identifiable information.
2. Types of personal information collected:
Code types of identification: C001 and C003
3. Period, locations, parties, and method of information use:
Period: The length of time necessary for the program to carry out its duties.
Parties: All entities related to the administration of the program, including administrative units of
schools, and supervisory or regulatory bodies.
Locations: The places in which the aforementioned parties are located.
Method: In accordance with the Personal Data Protection Act and other related laws and regulations.
In accordance with Article 3 of the Personal Data Protection Act, you shall be able to exercise the
following rights:
(1) the right to make an inquiry of and review your data;
(2) the right to request a copy of your data;
(3) the right to supplement or correct your data;
(4) the right to demand the cessation of the collection, processing, or use of your data;
(5) the right to erase your data. The program is not liable for any damages you may incur when your
exercise the aforementioned rights.
4. You are free to elect whether to provide your personal information; however, the project may be unable
to provide certain services if you refuse to give your information, or if the information you give is
false or incomplete.
5. In the event your personal information needs to be transferred to a third party for processing, the
project will exercise strict control and oversight over such processing by the third party.
6. You acknowledge this consent form is prepared in accordance with the Personal Data Protection Act and
other related laws and regulations. You also agree to the retaining of this consent form, which can be
retrieved at a later date if required.
Consent to provide personal information:
1. I have read and understood the aforementioned terms of this notice.
2. I agree to the collection, processing and use of my personal information by the program and other
government agencies that are providing administrative assistance.
